Your first visit to Advanced Foot & Ankle Associates establishes a vital foundation for our relationship with you. During the first visit, we will obtain important background information, such as your medical history and give you time to get to know your doctor. You'll find all the information you need for your first visit, such as directions to our office, practice hours, payment policies and more listed below. There is also background information about our committed staff and our first visit procedures. You can even save some time at your first visit by printing out and completing the patient forms in advance of your appointment.

Please review the Notice of Privacy Practices . This will be provided to you at your visit and will be signed upon your arrival.
What to expect
Please plan to arrive 20 minutes early for your first appointment and allow approximately 1 to 1.5 hours for us to thoroughly evaluate you. We ask that on the day of the appointment, do not apply lotions or creams to your lower legs or feet. Also, ladies please remove all nail polish prior to your visit.

- Form of payment for Copay (if applicable)
We participate with most insurance plans including Medicare. As a courtesy to you, we will submit the claim to your insurance company. We accept Visa, MasterCard, Care Credit , check or cash for any charges that are not covered by insurance.
